Instructions for Sending in Prescriptions for Mail Order:
- Complete “Patient” information on “Web Prescription Form”
- Make copies of actual prescriptions and the form (for your own file)
- Call Express Scripts Mail Order at 866-216-4540 to “set up your account” with Express Scripts (Have your credit card or your bank account information when you call)
- Send in the original “Web Prescription Order Form” and your original copies of the prescriptions to:

The Kentucky Conference Board of Pensions has selected Anthem Blue Cross and Blue Shield as our claims administrator effective September 1, 2010.

Full-time clergy and pastors serving in Kentucky Conference churches or who work directly for the Kentucky Conference are required to be in the health plan. Retired clergy and pastors who were serving full-time at the time of their retirement in the Kentucky (or Louisville) Conference are also eligible for the Medicare-supplement plan (given certain provisions).

The Board of Pensions is asking all current participants in the conference health plan to complete an enrollment form and return it to Robert Moorlach, Director of Benefit Services, 7400 Floydsburg Rd, Crestwood, KY  40014.

All current participants in the health care plan will be receiving a packet of information in the mail in the coming days.
