New Employee Paperwork

Bringing on new employees, both lay and clergy, can be an exciting time but often full of paperwork. Below are links to necessary paperwork when hiring new employees.


Forms to be completed by ALL new employees and retained by the church:

Form W-4: This form allows an employee to indicate to their employer their withholding needs for federal tax purposes. For lay employees, federal withholdings are based upon the marital status they check in BOX 3 and number of exemptions they indicate in BOX 5. For clergy, federal withholdings are based upon the dollar amount they indicate in BOX 6 (this is a per paycheck amount).

Form K-4: This form allows an employee to indicate to their employer their withholding needs for state tax purposes. For lay employees, state withholdings are based upon the number of exemptions they indicate in BOX 7. For clergy, state withholdings are based upon the dollar amount they indicate in BOX 8 (this is a per paycheck amount).

Form I-9: Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States, this includes clergy newly appointed to each local church. Please follow the instructions carefully on completing this form and for retention requirements.

Additional form to be completed by all CLERGY with new appointments:

UMPIP Contribution Agreement: This form must be completed each time a clergyperson receives a new appointment, even if they were making contributions at their previous appointment. Completed forms should be sent to Wespath Benefits & Investments.
